A sports physical is required for participation in athletics for students. Call (320) 589-7600 for appointments at SCMC Morris Clinic or use the link to select your appointment time. Starbuck Clinic will not have designated exam dates, but you will need to still call for an appointment.
Please note immunizations will not be addressed or given at sports physicals and $35 reduced payment is due at time of appointment. As a reminder, this is not a full physical or well-child exam, just a sports physical and only 15 min. Please have your sports physical paperwork completed prior to your appointment. This can be obtained from your school’s office or on your school website.
