2025-2028 Community Health Needs Assessment and Implementation Plan
In 2025, Stevens Community Medical Center (SCMC) collaborated with local public health agencies and surrounding communities to assess community health, identify priorities, and develop a plan to address those priorities over the next three years. An implementation plan will be solidified and published in the Spring of 2026.
Three health priorities emerged throughout our system:
- Mental and Behavioral Health
- Access and Affordability of Care
- Healthy Living Supports (Nutrition, Obesity, and Social Connection)
