How do I sign up for MyChart?

You can create a MyChart account by simply going to centracare.com/mychart, from there you can click ‘sign up now.’ Once verified, you will be guided through the steps to access your MyChart account.

How do I get to the clinic?

The Clinic entrance is on the south side of the campus. Please access The Clinic from South Street.

How do I get to the Urgent & Respiratory Clinic?

The Urgent & Respiratory Care clinic entrance is on the north side of the campus. Please access Urgent & Respiratory Care from First Street. Seperate parking is available at that entrance. 

Where do I park when visiting SCMC?

When visiting the clinic or hospital, parking is available on the south side of the campus. Access to parking is off of South Street. The Hospital is on the facility’s second floor. The elevator is available from the clinic lobby.

When coming directly to Urgent & Respiratory Care, use the separate entrance on the North side off of First Street (located next to ER).

How do I get a copy of my own or someone else’s medical record?

You can access it through your MyChart account. If you do not have a MyChart account you can simply create one by visiting centracare.com/mychart. You can also contact SCMC Release of Information (ROI).

Access to personal medical records is guaranteed under the Health Insurance Portability and Accountability Act (HIPAA) of 1996. In order to process your request for the release of your medical records, you must complete a form called the Authorization for Use and Disclosure of Patient Health Information. If the form is incomplete, it will be returned to you and no information will be released until it is properly completed. Please remember to sign and date the form. The date of signature must not pre-date treatment.

Is there a charge for a copy of my medical record

We provide current medical information (up to one year) free. If you need your entire medical record, there is a charge. Call (320) 589-7642 for an estimate of charges.

How long will it take to get a copy of my medical record?

We usually process requests within 7-10 business days.

Can I obtain copies of my spouse’s medical records?

The personal representative or next-of-kin may authorize the release of a deceased patient’s medical record.

How do I obtain a copy of my Birth Certificate

In Minnesota, call the Minnesota Department of Health at (651) 201-5970 or go to www.health.state.mn.us. You can also contact your county’s Vital Records Division, which is usually in the County Recorder’s Office.
