MHA award

Minnesota Hospital Association (MHA) recognizes Stevens Community Medical Center (SCMC) for Community Collaboration in Addressing Social Determinants of Health

St. Paul, MN – Forty hospital and health system teams and individuals from across Minnesota were recognized by MHA for their efforts to bolster the health care workforce and improve quality and safety in Minnesota’s nonprofit hospitals and health systems. The awards were presented at MHA’s Where Disciplines Meet: Putting Patient First Conference.

“There is amazing work being done every day to provide the highest quality care to our patients”, said Dr. Rahul Koranne, president and CEO of MHA. “It is not only incredibly important to honor the teams and individuals doing this work, but also valuable to convene these frontline leaders to share and learn from each other.”

SCMC received an award in the category of Community Collaboration in Addressing Social Determinants of Health because of their work and collaboration with the Stevens County Food Shelf

This past year, SCMC partnered with Horizon Public Health to obtain a grant from PrimeWest that would support reducing Food Insecurities, a measure found in Social Determinants of Health, linked to disparities. Non-perishable food items are available at no charge to SCMC clinic patients as needed. This program is not income based. This fall, Phase II of the program is expected to start and will provide fresh fruits, vegetables, and frozen meat on a monthly basis. SCMC’s goal is to help bridge the gap and improve access to healthier, nutritious food options.
