SCMC Launches Next Community Health Needs Assessment

Stevens Community Medical Center (SCMC) is beginning the process of conducting our next Community Health Needs Assessment (CHNA). As a nonprofit 501(c)(3) hospital in Minnesota, we are required to complete a CHNA every three years in accordance with federal and state guidelines. More importantly, this process helps us better understand the unique health needs of our community so we can provide care and resources that truly make a difference.

The CHNA gathers input from community members, local organizations, and healthcare professionals. This feedback helps us identify top health priorities, such as access to care, chronic disease prevention, mental health, transportation, child health, and other important issues facing residents of Stevens County and surrounding areas.

Your voice matters. We are asking community members of all ages and backgrounds to share their thoughts by filling out a short, anonymous survey. The survey is brief and only takes a few minutes to complete and will directly guide how SCMC shapes health programs, services, and partnerships in the years ahead.

All responses will remain anonymous. The final CHNA report, including identified priorities and action steps, will be shared publicly once the process is complete.
