2025-2028 community health needs assessment and implementation plan
During 2025, Stevens Community Medical Center (SCMC) worked with local public health and communities surrounding its hospital to assess community health, determine priorities and develop a plan to address those priorities in the next three years. An implementation plan will be solidified and published in Spring of 2026.
Three health priorities emerged throughout our system:
- Mental and Behavioral Health
- Access and Affordability of Care
- Healthy Living Supports (Nutrition, Obesity, and Social Connection)
